Endocrine and metabolic aspects of COVID-19

The paper presents the theoretical considerations on role of endocrine and metabolic alterations accompanying COVID-19 infection. These may be presumed basis following two observations. Firstly, virus SARS-CoV-2 responsible for infection uses an important renin–angiotensin system (RAS) element — angiotensin-converting enzyme 2 (ACE2) as a receptor protein entry into target cells and, in consequence, disturbs function main (circulating) renin–angiotensin–aldosterone (RAAS) local localized different tissues organs. binding to ACE2 leads downregulation this aftermath, excess angiotensin II aldosterone. Thus, later stage infection, beneficial effects ACEI ARB could presumed. It is hypothesized that RAS dysregulation adipose tissue cause negative obesity risk factor severe outcome Secondly, strongly depends age patient. Age-related hormonal deficiencies, especially those melatonin dehydroepiandrosterone, contribute morbidity/mortality older people. usefulness converting inhibitors/angiotensin 1 blockers (the latter only phases infection) adjuvant drugs probable but needs thorough clinical trials.
